In the large majority of cases, the cause traces to one of two specific components inside the tank failing to seal properly.

How the Tank’s Fill and Flush Mechanism Actually Works

When a toilet is flushed, a flapper valve at the bottom of the tank lifts, allowing the tank's stored water to drop rapidly into the bowl, and once the tank empties sufficiently, the flapper is designed to fall back into its seat and seal the opening, stopping further water from entering the bowl until the next flush.

At the same time, a fill valve, controlled by a float that rises with the water level, refills the tank to a preset level and then shuts off completely once that level is reached, meaning a properly functioning tank should reach a stable, full water level and then stop drawing any more water at all until it is flushed again.

An overflow tube built into the tank provides a mechanical backup to this shutoff function, draining any excess water directly into the bowl if the fill valve were to fail to shut off at all, which prevents the tank from actually overflowing onto the floor even during a fill valve malfunction.

What Actually Fails to Cause the Running Sound

A flapper that has degraded, warped, or accumulated mineral buildup along its sealing edge fails to form a complete seal against the valve seat, allowing a slow, continuous trickle of water from the tank into the bowl, which in turn triggers the fill valve to keep adding water to compensate for the level the tank can never actually maintain.

A fill valve that fails to shut off completely, sometimes due to a worn internal seal or a float that is misadjusted or physically obstructed, is the other common cause, and in this case the tank itself may be holding water properly while the fill valve simply continues running well past the point where the tank has already reached its intended level.

Where a Running Toilet Is Commonly Misdiagnosed

A running toilet is sometimes assumed to indicate a problem with the toilet's drain line or the home's broader plumbing system, when the mechanism is almost always contained entirely within the tank itself — the flapper and fill valve — rather than anything downstream in the drain or supply lines connected to the fixture.

Intermittent running, where the toilet briefly refills on its own every so often without being flushed, is also sometimes dismissed as normal, when this pattern specifically indicates a slow flapper leak severe enough that the tank periodically drops below the fill valve's trigger level, prompting a brief refill cycle.

A faint hissing sound without any visible movement of water is also sometimes assumed to be unrelated to the flapper or fill valve entirely, when a very slow, high-pressure leak through either component can produce exactly that sound without the more obvious sound of water audibly refilling the tank at any point during the entire ongoing course of the leak itself, from start to finish.

How the Failure Is Actually Diagnosed

A simple dye test, adding food coloring to the tank water and checking whether color appears in the bowl without flushing, directly confirms whether the flapper is failing to seal, since any color migrating from tank to bowl on its own indicates water passing the flapper's seal when it should be fully closed.

The water level relative to the overflow tube inside the tank indicates whether a fill valve is shutting off at the correct level — water at or above the overflow tube's opening indicates a fill valve failing to shut off in time, distinct from a flapper-related leak that would show a normal, stable fill level between flushes.

A running toilet's mechanism is almost always confined to two specific tank components failing to seal or shut off correctly — a narrow, well-understood failure mode, not a sign of a broader plumbing problem elsewhere in the house.
